JFS 07071State Lottery Prize Award Intercept Directive06/2006
 Note: OCS has created a new form in accordance with ORC section 3123.89 of the Revised Code.
This form shall be issued by a child support enforcement agency (CSEA) to the Ohio Lottery Commission (OLC) upon determining that an obligor who is the recipient of a lottery prize award is subject to a final and enforceable determination of default made under ORC sections 3123.01 to 3123.07.
