SUBJECT: Changes to the Child Care Enforcement Recommendation Forms

Background:

The Ohio Department of Job and Family Services (ODJFS) has revised the recommendation for suspension and recommendation for revocation/denial of license forms to update the documents to better align with rules 5101:2-12-05 and 5101:2-13-05 of the Ohio Administrative Code.

The following forms have been revised:

  • JFS 01161 “Recommendation for Suspension of Child Care License” has been revised to add questions regarding if the provider has a provider agreement, add the name of the program, the license type, the number of authorized children and include spacing and minor grammar changes.
  • JFS 01356 “Recommendation for Revocation/Denial of License of Family Child Care Home” has been revised to add questions regarding inspection types, add a place to note which years of inspections to include, add a place to specify if rule violations are serious or moderate risk, include a more detailed request to submit additional information with the form, change destination mailbox from regional to enforcement and include spacing and minor grammar changes.

County agency staff are required to use these forms when submitting recommendations for the revocation or the denial of the license of family child care homes or when recommending the license suspension of family child care homes.
