When the agency approves, denies, or delays an assistance
group's request for replacement food assistanceSNAP benefits, the assistance group shall be provided
written notice of the action.
(A)The notice
shall be mailed or personally delivered no less than ten calendar days from the date the loss was reported,
fifteen days if the issuance was made by certified or registered mail, or
within two workingbusiness
days after the agency receives the signed JFS 07222, "Statement Requesting
Replacement of Food AssistanceSupplemental Nutrition Assistance Program (SNAP)
Benefits (rev. 3/201010/2016),"
whichever is later.
If a notice of delay was issued, the agency shall issue an
approval or denial notice within two workingbusiness days from the date it receives the information
which caused the delay.
(B)The notice
shall contain: a clear and
understandable statement of the decision and the reasons for it, cite the
applicable regulations, explain the assistance group's right to and the method
of obtaining a county conference and a state hearing, and contain a telephone
number to call about free legal services.

(C)The JFS 07235 "Action Taken on Your Request for
Replacement of Food Assistance Benefits (rev. 12/2012)," or its
computer-generated equivalent, shall be used.
