This letter transmits a revised form that may be used by child advocacy centers to request start- up funding and a new form to request ongoing funding from the Ohio Children's Trust Fund. Also included are the program instructions for applying for start-up or ongoing funding. Applications for state fiscal year 2010 are due
February 27, 2009.
The following is a summary of the forms:
JFS 01474 "Application
for Child Advocacy Center Start-Up Funding" is a revised form to be used by a children's advocacy center to request start up funding from the Ohio Children's Trust Fund. This form is also being moved from the Family, Children and Adult Services Manual into the Children's Trust Fund Manual.
JFS 01473 "Application
for Child Advocacy Center Ongoing Funding" is a new form to be used by a children's advocacy center to request ongoing funding from the Ohio Children's Trust Fund.
