Upon request, the employer shall furnish to the director the names, social security numbers, and other information determined by the director to be necessary to determine whether the reason for separation unemployment of individuals employed at such factories, establishments or premises was due to a lockout or a labor dispute other than a lockout. The employer shall also furnish any information requested by the director with respect to the ending date of the dispute.
